Effectiveness of High-Dose Ketoconazole in Preventing Postoperative Penile Erection

Abstract

|
Nocturnal erections are androgen dependent. Ketoconazole, a synthetic imidazole dioxulane, reduces adrenal and testicular androgen production in man. To determine if ketoconazole can prevent postoperative spontaneous erections in patients
undergoing
penile surgery, we administered 400mg of the drug every 8 hours to 18 patients. No spontaneous erections were observed postoperatively in 16 patients. In 17 cases, serum testosterone concentrations were below 20ng/dL 24 to 28 hours affer
initiation
of
therapy. Once Ketoconazole was discontinued serum testosterone concentrations returned to baseline within 48 hours. Transient hepatotoxicity was observed in two patients, but liber function tests returned to normal within 2 weeks.
